Summary
We developed the Adults in Hospital change packages with the healthcare system in mind. They are informed by evidence, clinical and improvement knowledge and the expertise of people working across the system. This change package supports the implementation of the primary and secondary drivers for the falls programme. It also includes a measurement plan. Both of these documents can be found in the documents section below.
The Falls change package helps teams to:
- recognise and respond to deterioration early,
- develop patient-centred review and reassessment,
- improve outcomes through early intervention and timely management, and
- embed the essentials of safe care through evidence-based care.
