Summary
We developed the Adults in Hospital change packages with the healthcare system in mind. They are informed by evidence, clinical and improvement knowledge and the expertise of people working across the system. This change package supports the implementation of the primary and secondary drivers for the pressure ulcers programme. It also includes a measurement plan. Both of these documents can be found in the documents section below.
The pressure ulcers change package helps teams to:
- identify risk early and take action to prevent harm,
- provide timely assessment and treatment,
- work in partnership with people and carers to improve outcomes and reduce avoidable pressure ulcers, and
- embed the essentials of safe care through evidence-based care.
