This publication is the latest version of the SPSP Falls change package. It is an evidence-informed resource designed to support teams with improvement in numbers of patients who fall and patients who experience harm from a fall.
The package includes:
- A driver diagram, including a national aim, primary and secondary drivers
- Change ideas providing practical actions that teams can test and implement
- Tools, resources and signposting to supporting evidence
- A measurement framework that outlines outcome, process and balancing measures, and provides guidance on data collection and reporting
