Falls are a common cause of harm for people in hospital.
Falls change package
This change package provides an evidence-informed resource to support NHS boards with improvement work to reduce falls.
It includes the following:
- Driver diagram
- Change ideas
- Guides, tools and signposting to the supporting evidence and examples of good practice
- Guidance to support measurement
It should be used with the falls measurement framework.
The change package was co-designed by clinical, care, and quality improvement experts from across Scotland, and informed by the lived experience of people and their families.
