Summary
We developed the Adults in Hospital change packages with the healthcare system in mind. They are informed by evidence, clinical and improvement knowledge and the expertise of people working across the system. This change package supports the implementation of the primary and secondary drivers for the falls programme.
The falls change package helps teams to:
- deliver evidence-based care and the essentials of safe care
- create environments that promote safer mobility and encourage activity for the people who use them
- strengthen systems and processes to reduce falls and improve outcomes, and
- provide care that is people led and based on individual need.
