Summary
We developed the Adults in Hospital change packages with the healthcare system in mind. They are informed by evidence, clinical and improvement knowledge and the expertise of people working across the system. This change package supports the implementation of the primary and secondary drivers for the pressure ulcers programme.
The pressure ulcers change package helps teams to:
- deliver evidence-based care and the essentials of safe care
- identify risk early and take action to prevent harm
- provide timely assessment and treatment, and
- support the assessment process and increase awareness of ‘prevention’ with people and carers to improve outcomes and reduce avoidable pressure ulcers.
