  1. The provider will operate an independent clinic.       
  2. The service must only operate between the hours of 0700 and 2100 Monday to Sunday, except for the provision of emergency care.   
  3. The provider must only allow non-surgical treatments, namely: [facial and body aesthetics treatments], [blood tests], [laser and IPL treatments], [general non-surgical procedures], [gynaecology non-surgical treatments], [GP consultation/assessment/treatment], [specialist consultation/assessment/treatment] and minor surgical procedures, namely: [general minor surgical procedures], [thread lift procedures], [laser minor surgical procedures], [male genital minor surgical procedures].  
  4. The provider must not allow the provision of dental treatments (including dental hygiene treatments, dental implants, tooth whitening and orthodontics).   
  5. The provider must not allow any ionising radiation exposures to be carried out.  
  6. The provider must not allow the prescribing of cannabis-based medicinal products.  
  7. The provider must not allow any treatments or procedures to be carried out using any form of sedation or general anaesthesia.  
  8. The provider will treat service users aged from birth for GP consultation/assessment/treatment, dermatology consultation/assessment/ treatment and 18 or over for all other treatments or procedures.
