This resource is a source of information and evidence for Integration Authorities and NHS Boards in Scotland. It looks at the provision of “Hospital at Home” services.
It reviews the published evidence on the effectiveness and safety of Hospital at Home initiatives. It is specifically for older people living with frailty. It shares learning from existing services across Scotland. It is intended to assist in local and regional planning for acute and specialist services. It will support people, who would ordinarily require admission to acute hospital, to receive treatment at their home.
