Continuity of Care and Frailty Cluster Session – Opportunities beyond the DES

On 27 October and 4 November we held a cluster sharing and learning session for Cluster Quality Leads (CQLs) and Practice Quality Leads (PQLs) to share experiences on initiating a QI approach to the Continuity of Care and Frailty Directed Enhanced Service (DES).  

These sessions were an opportunity to: 

• Hear examples of how CQLs have approached this in their clusters,
• Share opportunities, approaches, successes, challenges and how they were/could be overcome, and 
• Generate change ideas that could be tested to improve continuity of care approached in frailty and also in other areas.
