This change package provides an evidence-informed resource to support teams in the prevention of acquired pressure ulcers developed in a care setting.
It includes the following:
- driver diagram
- change ideas
- guides, tools and signposting to the supporting evidence and examples of good practice
- guidance to support measurement
It should be used with the pressure ulcers measurement framework .
The change package was co-designed by clinical, care, and quality improvement experts from across Scotland, and informed by the lived experience of people and their families.

The measures contained in this framework will assist teams to measure key changes in the pressure ulcer change package.
