Deteriorating patient change package
This change package provides an evidence-informed resource to support teams with improvement in the early recognition and structured response to patient deterioration.
It includes the following:
- driver diagram
- change ideas
- guides, tools and signposting to the supporting evidence and examples of good practice
- guidance to support measurement
The change package was co-designed by clinical, care, and quality improvement experts from across Scotland, and informed by the lived experience of people and their families. It considers the SIGN Care of the Deteriorating Patient Guideline (2023).
Deteriorating patient and sepsis measurement framework
Measures are essential to help teams to learn if the changes they are making are leading to an improvement.
This measurement framework is designed for use with the SPSP Deteriorating Patient and SPSP Sepsis driver diagrams and change packages.
