What is a delayed discharge?
A delayed discharge happens when someone is ready to leave hospital but cannot do so because:
- the right support is not yet available at home or in the community
- a suitable care placement has not been found
- discharge planning is not yet complete
Improving discharges
By improving the way discharges are planned and supported, we can:
- make sure people spend less time in hospital
- help them return to their own home or community sooner
- ensure hospital care is available for those who need it most
