Delayed Discharge Improvement Work

We want to make it easier for people with mental health needs, learning disabilities, and adults with incapacity to move on from hospital when they’re ready. Too often, people stay in hospital longer than necessary. This is because the right support or accommodation isn’t yet in place.

We are working with NHS Boards and Health and Social Care Partnerships across Scotland to:

  • improve discharge planning and communication
  • strengthen community support
  • reduce unnecessary delays

We are supporting this change by:

  • listening to people with lived experience, families and staff
  • testing and implementing practical improvements that make a difference
  • sharing learning across local and national teams
