Conditions of registration

1. The provider will operate an independent medical agency.
2. The service must only operate between the hours of 0700 and 2100 Monday to Sunday. 
3. The provider must only allow non-surgical treatments, namely [specialist consultation/assessment/treatment] and [mental health consultation/assessment/treatment].
4. The provider must not allow the face-to-face treatment of patients in any fixed premises. 
5. The provider must not allow the prescribing of cannabis-based medicinal products. 
6. The provider must not prescribe medication for patients introduced by an unregulated third-party organisation whereby the established governance structures prohibit, or indirectly restrict, the service being able to communicate directly, either virtual or in person, with the service user.
7. The provider must only treat service users aged [18 or over].  
8. The provider must not allow children under the age of 18 to be present during any consultations, treatments or procedures.
9. The provider must ensure that staff possess the appropriate skills, knowledge and experience and that they are provided in sufficient numbers to ensure the safety and welfare of patients, the public and wider workforce. 
10. The provider must ensure that staff requiring to be registered by a professional regulating body are actively registered.
