Summary

Reducing Harm, Improving Care found that people with complex care needs need services that are more flexible. We found that no one person was responsible for overseeing or coordinating care, even when care needs were complex and involved several services. People told us that some health conditions went untreated because they had to focus on their most urgent need while coping with difficult circumstances.

Our findings showed that many people had to manage their own care, even when their personal situation made this difficult. It shows how people can fall through gaps in the system. They also highlight the need to improve how care is delivered in difficult circumstances to reduce drug-related harms and improve outcomes.
