Focus on Dementia is a national improvement programme. It improves quality of care for people with dementia. We work with health and social care teams from across hospital and community settings, including the third sector.
We aim to improve access to, and experience of, health and social care to ensure care is:
- person led
- coordinated
- leads to improve outcomes.
We use quality improvement methodologies to:
- improve care experience for people with dementia
- improve access to support in the community for people with dementia
- improve care co-ordination in the community for people with dementia
- reduce avoidable admissions to hospital for people with dementia living in care homes
- reduce time people with dementia spend in hospital.
