Multidisciplinary team (MDT) working is a national improvement programme. It has been established to support the further development of MDTs in primary care. This was one of the ambitions outlined in the General Medical Services (GMS) contract (2018).
The aim of the programme is to ‘improve patient and staff experience by strengthening MDT working in primary care‘.
An improvement collaborative approach will be adopted to support teams over a 12-month period. Participating teams will be supported to assess, identify and implement changes based on their local priorities. Potential improvements include:
- safer, more coordinated and person-centred care for patients
- clearer ways of working across the MDT, and
- improved team culture, collaboration and communication.
We are looking for teams interested in joining the programme now.
