Focus on Dementia is a national quality improvement team. We aim to improve access to and experience of person-led, co-ordinated health and social care for people living with dementia. We apply quality improvement methodology to drive improvements by bringing together teams from:
- health
- social care
- third sector
Our latest programme will focus on:
- improving access to, and the quality and experience of post-diagnostic support (PDS)
- improving care co-ordination, for those with more complex needs
- improving specific areas of practice such as personal planning
