Summary
On Wednesday 23 and Tuesday 29 September 2026, the Scottish Government, the Royal College of General Practitioners Scotland and Healthcare Improvement Scotland hosted an online webinar on the Frailty Directed Enhanced Service (DES) and continuity of care.
The webinar was aimed at Frailty Leads from GP practices across Scotland, as well as members of the multidisciplinary team with an interest in frailty and improving care for older people.
The webinar highlighted the requirements of the Frailty DES and explored the importance of continuity of care in supporting people living with frailty. It also shared evidence-based approaches for assessing and managing frailty within general practice and participants were introduced to the modified SLICC measurement tool and dashboard and learned about the resources and support available to help implement effective, person-centred frailty care within practice teams.
Presentation slides are available on request by contacting his.frailty@nhs.scot
