Focus on Frailty is a national improvement programme. Our work focuses on older people living with frailty. We aim to improve access to, and experience of, health and social care for this group that is:
- person led
- coordinated
We bring together health and social care teams from:
- primary care
- community care
- acute care
The programme applies quality improvement methodology to drive improvement.
Our programme for 2026-2027 focuses on:
- Proactive community services to support people to live well in the community and avoid hospital admission
- Acute hospital frailty services
- Integration and pathways between community and hospital
