The Coming Home Peer Support Network supports the Coming Home Implementation Plan (2022).
The network
The network helps reduce out-of-area placements by improving local capacity and expertise. It connects professionals across Scotland who have responsibility for planning and delivering services for people with learning disabilities and complex support needs.
The Coming Home Peer Support Network aims to:
- Connect peers across Health and Social Care Partnerships (HSCPs) and lead agencies
- Facilitate open and honest conversations about service planning and delivery
- Share learning and good practice from across Scotland
Webinars and peer learning
We host regular webinars. They explore key topics related to the Scottish Government Coming Home Implementation Plan (2022). These sessions include opportunities for small-group discussion, peer learning, and reflection.
We share the resources through our Microsoft Teams channel. We also host smaller ‘surgery-style’ meetings between webinars for practical peer discussions.
