Our support is for all types of adult hospital at home services, this includes general acute adult services and specialist services, such as older people/frailty, respiratory or heart failure.
The current focus is expanding capacity of hospital at home services to contribute towards the Scottish Government aim to reach 2,000 hospital at home and Outpatient Parenteral Antibiotic Therapy (OPAT) beds by December 2026.
We do this by working with services to:
- Set-up appropriate referral pathways
- Develop effective clinical and non-clinical processes
- Enable workforce planning
- Support senior leadership and good governance, and
- Share evidence and experience.
Programme support
We provide participating services with:
- A set of guiding principles to support existing and new services
- Peer to peer learning for example through MDT site visits, regular coaching calls and access to other specialist advice.
- Access to information and resources
- Implementation guidance and advice
- Data Measurement and impact
- Learning from tailored sessions, connections and feedback for example, NHS boards/HSPC’s trying to establish new services or significantly expand the scope of geography or pathways.

Adult Hospital at home community
We facilitate an active network of hospital at home practitioners for peer-to-peer support and sharing of learning. This includes:
- an active MS Teams channel
- regular online networking sessions
- national events, and
- a large library of practical resources such as pathways, standard operation procedures, job descriptions.
