Getting involved in the reducing stress and distress improvement programme (2026) application questions

These are the questions included within the application form.

1. Are you applying as a hospital ward or care home?

2. What is the name and location of your hospital ward/care home?

  • Hospital ward – provide the name of your hospital and in which NHS board it is located.
  • Care home – provide the name of your care home and if you know which NHS board or health and social care partnership it is located.

3. What is the name of your care home provider?

Provide the name of your care home provider if you are part of a care home group.

5. Tell us about the type of ward and service you provide.

  • Hospital ward – provide a description of your ward and the service you provide day to day. This should include the needs and conditions of patients you regularly support, number of beds, the environment and any multidisciplinary team providing care. 
  • Care home – provide a description of your care home and the service you provide day to day. Please include the needs and conditions of residents living with dementia and/or have cognitive impairment you regularly support, number of beds, the environment and the multidisciplinary team providing support.

6. Tell us about your team and the people who are going to be involved in your project.

  • Your team should have a named project lead and at least another two key team members who will be actively participating in online learning sessions and driving forward the project within your ward. The project lead should be a frontline member of staff.
  • Hospital ward – your team should be multidisciplinary and can include nurses, allied health professionals and ward support staff. It is also helpful if your team includes dementia consultant/specialist.
  • Care home – your team should include care home manager, staff nurse/senior care worker, carer/domestic staff, activity co-ordinators and/or allied health professionals. It is also helpful if your team includes a dementia ambassador or member of your local care home support/liaison team.

7. Tell us your reason for applying and any current challenges in supporting stress and distress in the care of people living with dementia in your service.

  • The focus of this programme is to support you to make changes to your service. We will support you to use different tools to understand your service to help you make improvements. Your answer should include a clear reason for joining the improvement programme and will highlight a commitment to improving your service for people living with dementia. Are there any challenges you are currently experiencing when supporting people living with dementia who are upset, anxious or distressed? Please explain.
  • Hospital ward – This could include hospital safety ward round reports and any plans or changes you have put in place after feedback from others and how these changes improved the care you provided.
  • Care home – This could include inspection reports and any plans or changes you have put in place after inspections or feedback from others and how these changes improved the care you provided.

8. Do you have any local quality improvement, or do you have support to help you gather information in your service during this programme?

  • If your team know of any local quality improvement or data support in your NHS board who can support your team, let us know. If you do not have access to them, please tell us. This will not affect the outcome of your application, but it gives us a clear idea of how much support we will allocate to your team.

9. Tell us about any recent improvements your team has put in place, or priorities you have identified to improve the experience of people living with dementia who experience stress and distress. How did you identify them?

  • We want to know about any activity you are already carrying out to improve your service and how you went about identifying them.

10. What local or national networks are available to you in relation to people living with dementia and how do you share your dementia care and support and how have you shared your learning with others? If no, please type ‘no’.

  • We are keen to hear if you are part of any local or national networks that support sharing of learning.
  • This will not affect the outcome of your application but it gives us a clear idea of how we can signpost you to networks that are beneficial for your service.
  • Hospital ward – This could include networks within your hospital, locally with other healthcare services, national networks or learning systems like Dementia Consultant Network, Dementia Specialist Improvement Leads (DSIL), Dementia Champions Network, AHP Dementia Forum or Dementia Learning System.
  • Care home – This could include within your care home group, locally with other care homes or networks or learning systems for example use of SSSC and NES Turas webpages, Care Inspectorate Dementia Hub, Dementia Ambassadors Network or Dementia Consultant Network.

11. Has your team previously or is currently involved in other Healthcare Improvement Scotland, Care Inspectorate improvement programmes or HSCP/health board supported improvement work? If yes, please provide information on the improvement programme/s or work.

  • Your care home or ward may be involved either previously or currently in other programmes of improvement work. Please tell us of any improvement work and when this is scheduled to end.

  • The project lead should be the key contact who will be responsible for leading the team throughout the programme. We will liaise with this person with any clarifying questions and information on the outcome of the selection process.

  • The executive sponsor is a senior manager, such as care home group operations manager, nurse director who will provide strategic oversight and actively champion the work of the team in the ward. The sponsor will not be actively participating or leading the programme.

  • Tell us this executive sponsor’s full email address and telephone number so that there is an alternative way of communication should we need it.

18. I agree to fulfil the key responsibilities of participating in the Focus on Dementia: reducing stress and distress improvement programme.

Hospital and care home teams across Scotland will participate in the programme. These teams will also fulfil key responsibilities to ensure their participation leads to improved outcomes for people living with dementia. By submitting the application form, you are agreeing to fulfil your responsibilities as a participating team.

We will: Advocate and promote programme with Scottish Government colleagues and media, lead programme delivery and management, provide resources, provide practical improvement support, review and evaluate programme outcomes, facilitate and share learning peer to peer and nationally and measure and evaluate progress and impact.

Your commitment to us:

You will: Identify a team to engage in this work who can commit to a minimum of 2 days each month over the 12-month period, confirm the names and contact details of team members, attend all programme (coaching and learning) sessions, collect and submit data, agree and test change ideas, participate in evaluation, share learning and resources and ensure there are contingencies in place to make sure that the project progresses as planned.
