Create the conditions

Preparation period

Actions to be taken during the preparation period include:

  • application and team selection
  • Memorandum of Understanding completion
  • initial data collation
  • capacity planning

Identify areas of focus

Week 1 and 2: Mobilise

Actions to be taken during weeks one and two include:

  • introductory coaching session
  • HIS review and analysis of feedback
  • reviewing information provided by HIS
  • select pathway or subspecialty of focus
  • agree plan for additional data collection

Weeks 3 – 6: Understanding and setting aims

Actions to be taken during weeks three to six include:

  • use data collection tools provided by HIS
  • continue coaching sessions to understand demand and activity
  • facilitated session to develop a process map
  • collate and prioritise opportunities for improvement
  • develop a measurable aim statement
  • select change ideas
  • agree project measures

Test, refine and adopt

Weeks 7 – 18: Testing and adoption

Actions to be taken during weeks seven to 18 include:

  • design tests of change
  • test changes using PDSA cycles
  • continuous data collection and sharing data with HIS
  • regular project sessions to progress testing, adapting and refining changes
  • coaching and data support from HIS

Sustain and share

Week 20: Sustain

Actions to be taken during and from week 20 include:

  • attend session to share learning with other specialties
  • agree final actions needed to sustain improvement
